DOCUMENTATION GUIDELINES FOR STUDENTS WITH DISABILITIES

The following suggested guidelines are what the professional staff with Disability Support Services use, in conjunction with the request for services process, to set up appropriate accommodations when registering students with our office. These guidelines are based on suggested disability guidelines established by the Association of Higher Education and Disability (AHEAD). Please select the documentation guidelines appropriate for the disability diagnosed.

AD/HD Documentation Guidelines

Hearing Impairment Documentation Guidelines

Learning Disabilities Documentation Guidelines

Physical Impairment Documentation Guidelines

Psychological Disabilities Documentation Guidelines

Visual Impairment Documentation Guidelines

Once Disability Support Services has received documentation, a staff member will meet with the student to review the documentation and complete the registration process which will include setting up accommodations that are supported by the documentation. Each student’s situation is evaluated on a case by case basis. If there are questions or concerns about the documentation guidelines that Disability Support Services follows, please contact Disability Support Services at 573-651-2273.
