GUIDELINES FOR REGISTERING AS A STUDENT WITH A DISABILITY

1. Once accepted into the university, enroll in courses.

2. If you have enrolled in courses on the main campus, make an appointment with the professional staff at Disability Support Services. If you have enrolled in online courses or courses at the Regional Campuses, please download and fill out the Initial Request for Services. Please return this form to Disability Support Services.

3. Documentation may be brought in to the office, or mailed in prior to the appointment. Please see Documentation Guidelines for more information about what kind of documentation you will need to be able to provide. If documentation is sent to Disability Support Services, please complete and return the Initial Request for Services to our office.

4. Students should keep in mind that accommodations are not retroactive; therefore, it is in the student's best interest to provide documentation in a timely manner and get registered as soon as possible after recognizing that services may be appropriate.

5. After documentation has been reviewed and accepted, the student will complete registration forms for disability services. Please keep in mind that a diagnosis alone does not qualify a student for services. There must be an indication that due to the impact of what has been diagnosed, the student's functioning is substantially limited.

6. Once the student has become registered for disability services, Disability Support Services will send Faculty Notifications to each of the student's professor to indicate that the student is eligible for accommodations and to detail what those accommodations are. (Specific disability information is not disclosed in these notices.)

7. The student should make appointments with each of his or her professor's to discuss accommodations. If the student does not acknowledge to his or her professor that the student wants to use their accommodations, the professor is not obligated to provide the accommodations.

8. Students will be required to sign a release for each semester that they would like their professors notified. Without that signed release, Disability Support Services will not send the Faculty Notifications.
