Once you have registered for your classes, it is time to give some thought to the disability accommodations memo that DSS  has been sending to your faculty.  Our policy requires that we have your written permission for each term  you wish to have your faculty notification memo sent. This also provides an opportunity for you to revise and update your memo as well. 

Please complete, sign and return this memo if you would like to have your faculty notification memo sent for the Fall, Spring, and/or Summer semesters. You may mark one, two or all three of the semesters, depending upon your academic plans for the next school year. If you wish to discuss your current accommodations or make any changes to your accommodations, please call and speak with one of the disability service providers or make an appointment to come and discuss your needs. 

You may return this form to Room 302 in the University Center, send it through campus mail to MS1300, or mail it to: Disability Support Services,  Southeast Missouri State University, MS 1300, One University Plaza, Cape Girardeau, MO 63701. You may also submit this form electronically by completing the information requested below and clicking submit.  Please do this as quickly as possible in order that we may notify your faculty prior to the first day of class.
